Exciting Changes to Dialyze Direct’s Admission Criteria

We are excited to share a significant policy change that will expand access to our home dialysis program. A large portion of our patient population is new to dialysis and currently diagnosed with Acute Kidney Injury (AKI). Over the past four years, we have worked closely with CMS to advocate for including AKI patients in the home dialysis program.

Starting January 1, 2025, an ESRD diagnosis will no longer be required to admit Medicare patients to our program. We can now admit to our home dialysis program all patients with kidney failure for whatever reason. This breakthrough change will enable you to discharge more Medicare patients to our partnered Skilled Nursing Facilities (SNFs), where they will have access to the comprehensive rehabilitative services offered in these environments.

We continue to enhance our intake processes to ensure a smooth transition for your patients. Currently, most referrals are approved within two hours, a significant improvement that helps our local case managers discharge patients more efficiently.

Thank you for your continued partnership. Please don’t hesitate to contact our Director of Patient Services, Brian Kelsey, if you have any questions or need further assistance.

Warm regards,

Dialyze Direct
