How we work to improve the referral process for CM and onsite dialysis needs:
- We partner with over 165 SNF facilities in 12 states.
- Our location finder can help you find SNF facilities that offer additional patient care services, such as vent and trach support, eliminating guesswork for you and your staff.
- Our referral review process is an industry-standard setting, supporting rapid clinical and financial reviews and approval notifications for placement.
- We communicate directly with CM and SW to help with placement when needed.
- Our Intake staff works closely with SNF admissions coordinators, supporting their facility-specific needs.
- We are in network with multiple Commercial and Managed Care payor sources, including Aetna, Humana, and Cigna, and work with many others out of network for acceptance.
- We offer patients conventional (3 days/week) and MFD (5 days/week) dialysis therapy models at specific locations to promote recovery and focus on the rehab needs of our patients, limiting complications, infections, and return hospitalizations.
